I've seen a few like that. There are a few reasons why a loco opperating at a boiler pressure of maybe 225 psi should blow. Over pressure, obviously, a faulty safty valve and poor maintenance being common.

Yeah, commenters in the video I posted above had relatives who worked on steam locomotives said the same thing.  Some of them also mentioned that their relatives were always nervous anytime they had to work with an engineer who kept the water level low...

In most cases, depending on the loco, water level was usually somewhere between 60 and 70%. Obviously, most of the boiler was filled with boiler tubes through which hot combustion gases passed thus heating the water. Surprisingly, a lot of people think that the boiler was just a cylinder full of water, but the whole process is a bit more complex.

Was talking with a friend who was a nuke on the carrier Dwight Eisenhower.  There was a stupendous amount of duplication for the engineering systems.  This included a small boiler to provide low pressure steam (150 lb) for secondary systems like running the laundry.
This boiler never was used and there was only one E5 Boiler Tender in the crew.  As part of the PMS, Preventive Maintenance System, the boiler was lit off and run once every six months.  The BT once forgot that prior to light off, the boiler fan was to clear all build up of fumes.  There was fumes left in the combustion chamber.  When the ignition stick went in there was a Whoosh and blow back. 
Took about four months to grow his eyebrows back.
